Please note that all appointments must be confirmed by yourself during the week prior to your appointment. This may be done by confirming at the time of your automated confirmation call by our office 2-4 days prior to your scheduled visit. If your appointment is not confirmed 24 hrs prior to the visit time, your appointment will be cancelled and another patient will recieve your appointment time.

On your first visit, you will be asked to come in early to fill out forms. These include a patient information sheet and patient history sheet. You will also be provided with privacy information relating to the Health Insurance Portability and Accountability Act, passed in 1996, with a signature page to acknowledge that you have received this information.

Please plan to bring copies of all relevant previous medical evaluations, all of your current medications, insurance information, laboratory reports, x-ray reports, CAT scan reports,. and MR scan reports with you for your neurologist appointment.

You are expected to provide co-pays or full payment at the time of your office visit as required by your insurance plan. For your convenience, we accept cash, check, or major credit cards.

 

Preparing for your EEG test

Adjust your sleeping schedule as directed by your physicain.

Wash your hair thoroughly. No conditioners, gels, or hair spray.

You may eat before being tested.

Take your normal medication.

Preparing for your EMG/NCS study

Keep extremities warm by wearing heavy socks, closed shoes, and gloves.

No lotions/creams to the skin on the day of the test
